Care Management
AACF Community Senior Care services begins by:
- Conducting a conference with the client and/or family members regarding in-home care.
- Evaluating what care is needed by a visit to the home of the client/client family.
- Providing a written report to the client, the primary care physician, and the family.
- Arranging for the in home companion to assist with meal preparation, shopping, transportation, household management or to obtain medical equipment.
- Coordinating care needs daily, weekly or monthly to maintain safety and satisfaction for the client.
